Sport Medical Forms
Any student interested in trying out and/or competing on an MCC intercollegiate athletics team is required to provide sports medical documents before they are permitted to try out, practice, or compete.
All student-athletes are required to submit the following:
- Proof of Insurance
- Athlete History Form
- Sport Physical
- Immunization Records (Measel, Mumps, Rubella, TDap, Meningitis)
- COVID Vaccination Verification, Exemption, or Temporary Approval
All new and returning student-athletes will be required to complete and submit the above sports medical documents electronically via the online Student Health Services Portal. In order to access the student-athlete health service portal a student must have the following -
- Accepted at Monroe Community College
- Received a Student ID # or M#
- Set-up and have access to your MCC Username and PW - same as your MCC email login and PW
- Coded as an MCC Student-Athlete - please speak with your coach to ensure you are listed as a student-athlete in our system.

INSURANCE REQUIREMENT: ALL STUDENT-ATHLETES
- Student-athletes are required to have a comprehensive insurance plan while participating in, or trying out for, an athletic team.
- A copy of both sides of your insurance card needs to be uploaded in the student health services portal in order for you to be cleared for tryouts, practice, or competition.
- All international students are automatically enrolled and billed for the Student Health Insurance Plan, unless proof of comparable coverage is provided.

COVID VACCINATION REQUIREMENTS
The State University of New York (SUNY) has announced that the COVID-19 vaccine will no longer be mandated for students to attend SUNY's 64 campuses. This updated COVID-19 guidance will become effective Summer 2023. This coincides with the official end of the national public health emergency.
Moving forward, COVID-19 vaccinations will be strongly encouraged for students, which is consistent with other viruses including influenza and mpox. Faculty and staff will also be encouraged to stay up to date on vaccinations.
SUNY will continuously examine COVID-19 data and update its policy as needed based on local conditions or in response to requirements imposed by federal, state, or local authorities. MCC will also continue to monitor local conditions carefully and make changes as appropriate.
